Key Responsibilities- Conduct initial evaluations to assess patients' psychosocial status and identify factors that may impact their care plan.
- Develop and implement individualized care plans in collaboration with the patient's healthcare team.
- Provide ongoing support and guidance to patients and their families, addressing emotional and social factors that may affect their health and well-being.
- Collaborate with the patient's healthcare team to ensure seamless care coordination and communication.
- Document patient progress and outcomes, maintaining accurate and comprehensive clinical records.
- Participate in quality improvement initiatives and contribute to the development of best practices in medical social work.
- Master's degree in Social Work from a Council on Social Work Education (CSWE) accredited program.
- Current licensure as a Clinical Social Worker (LCSW) in the state of Florida.
- Basic Life Support (BLS) certification.
- Valid driver's license and reliable transportation.
- At least two years of experience in a healthcare setting, with a minimum of one year in a medical social work role.
