Job Summary

Trinity Health At Home is seeking a compassionate and skilled Home Health Aide to provide one-to-one care to our patients in their homes. As a Home Health Aide, you will work closely with our interdisciplinary team to deliver high-quality, patient-centered care that meets the unique needs of each individual.

Key Responsibilities
  • Assist patients with personal care activities, including bathing, dressing, and grooming
  • Provide support with household tasks, such as meal preparation and light housekeeping
  • Perform range of motion exercises and other simple procedures as assigned by a registered nurse
  • Document patient progress and report any changes in condition to the registered nurse
Requirements
  • Completion of a federally compliant training program for Home Health Aides
  • Successful completion of a competency evaluation program
  • One year of experience as a Home Health Aide in an institution, home care, or hospice setting
  • Ability to work with minimal direct supervision and maintain a professional relationship with patients and their families
