As an Independent Optometrist at Sam's Club, you will have the autonomy to establish your own practice, set your own schedule, and control your patient schedule. You will be responsible for providing eye and vision care services to members, as well as general health and well-being services. This is a unique opportunity to build your own business and work independently, while still having access to resources and support from Walmart.

Key Responsibilities
  • Provide eye and vision care services to members
  • Establish your own schedule and control your patient schedule
  • Set your own fee schedule and keep 100% of exam/professional fees
  • Have access to equipment and instrumentation, as well as administrative and/or Opt-Tech support services (if state allows)
Requirements
  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school (for certain locations)
  • Valid state ophthalmology license (for certain locations)
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
