Key Responsibilities:- Provide Clinical Expertise: Utilize your advanced knowledge of pharmacology and clinical pharmacy practice to provide evidence-based pharmaceutical care services to patients.
- Collaborate with Healthcare Team: Work closely with physicians, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans.
- Conduct Patient Assessments: Conduct thorough patient assessments to identify medication-related problems and develop strategies to resolve them.
- Develop and Implement Treatment Plans: Develop and implement treatment plans that are tailored to the individual needs of each patient.
- Monitor and Evaluate Patient Outcomes: Continuously monitor and evaluate patient outcomes to ensure that treatment plans are effective and make adjustments as needed.
- Provide Education and Support: Provide education and support to patients and their families to promote medication adherence and optimize health outcomes.
- Stay Current with Best Practices: Stay current with the latest research and best practices in clinical pharmacy to ensure that you are providing the highest level of care to our patients.
- Education: Bachelor of Science in Pharmacy or Doctor of Pharmacy degree required.
- Experience: Successful completion of a clinical pharmacy residency or equivalent experience in direct patient care delivery required.
- Licensure: Pharmacist license in California required at hire.
- Certifications: Basic Life Support certification from the American Heart Association required.
- Comprehensive knowledge of current pharmacological and biopharmaceutical principles.
- Working knowledge of aseptic technique and preparation of sterile IV products and chemotherapy agents.
- Understanding of drug utilization review and drug usage evaluation.
- Knowledge of Federal and State laws and regulations regarding the provision of pharmaceutical services.
- Excellent verbal and written communication skills.
