Key Responsibilities:
- Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care.
- Utilize standardized assessments and evidence-based practice to support clinical interventions.
- Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders.
- Assess patient progress and adjust treatments accordingly.
- Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
