Job Description

The Patient Care Coordinator provides general aspects of patient care that meet physical and/or psychosocial needs, within the nursing assistant's scope of practice. Responsibilities include assisting with admission, transfer, and discharge of patients, communicating effectively with the RN or LPN about observed changes in patient status and responses, and notifying licensed personnel when a patient complains of pain.

Required Skills and Qualifications

  • Must be able to read and communicate effectively in English
  • Basic computer skills are necessary
  • High school graduate or equivalent; successful completion of certified nursing assistant program is preferred but not required
  • Active Certified Nurse Assistant certification upon hire or receive certification within the new employee introductory orientation period and maintain clinical competency during employment with ECHN
  • Must have certification in Basic Cardiac Life Support upon hire or receive certification within the new employee introductory orientation period and maintain clinical competency during employment with ECHN
