We are seeking a highly skilled Clinical Logistics Nurse to join our team at Core Clinical Management LLC. As a Clinical Logistics Nurse, you will play a critical role in improving the quality and efficiency of care for hospitalized patients.
Key Responsibilities:
- Process Improvement: Collaborate with Medical Directors to develop and implement site-specific workflows that achieve program goals and performance targets, aligning with hospital and Core priorities.
- Process Optimization: Partner with the Medical Director to implement Core's processes and lead process optimization efforts.
- Status Updates: Provide timely status updates of implementation, barrier resolution, and action plans.
- Clinical Performance Objectives: Understand, clearly articulate, and promote the adoption of hospital and Core clinical performance objectives with the physician/APP team.
- Clinician Engagement: Engage and educate clinician team(s) related to clinical performance initiatives, goals, and measurement.
- Clinical Performance Activities: Oversee and facilitate clinical performance activities, potentially across multiple service lines.
- Care Team Collaboration: Participate in daily workflows by collaborating with various care team members, advocating for patient goals in the care plan, and enhancing communication throughout the care team as the patient moves through their hospital stay.
- Facilitation and Coordination: Facilitate coordination between the hospital medicine team and hospital departments to ensure the delivery of workflows related to throughput, discharge facilitation, and transitions of care, readmission prevention, patient experience, and timely physician response to documentation queries.
- Patient and Family Education: Coordinate patient and family education to ensure understanding of the disease process, next-level-of-care (NLOC) options, and community resources.
- End-of-Life Care Planning: Coordinate and facilitate follow-up and goals of care planning for patients identified to be in the end-of-life stage of their disease process.
- Readmission Prevention: Ensure a process for scheduling community provider follow-up post-discharge for patients determined to be elevated risk for readmission.
- Program Evaluation: Participate in initiative-taking evaluation of program metrics and initiate additional investigation of drivers when targets are met.
- Performance Data Sharing: Collaborate with the Medical Director, DCO, DOO, and hospital leaders to share performance data with the physician team to drive continuous improvement.
- Process Evaluation and Improvement: Continually evaluate processes, tools, and workflows to identify opportunities for improvement.
- Committee Participation: Participate in hospital committees and special projects as appropriate.
- New Physician Orientation: Orient new physicians to Core's processes, quality, and patient experience initiatives.
- Team Collaboration: Collaborate with Medical Director, DOO, and DCO to contribute to team meeting agendas.
- Point of Contact: Serve as a point of contact for hospital staff (e.g., issues related to clinical processes and triage for other potential issues related to hospital medicine team).
- Educational Forums: Coordinate educational forums as needed.
Requirements:
- Clinical Knowledge and Experience: Current RN licensure in good standing, in the state applicable to the position, and strong clinical knowledge and experience in the Acute Care Hospital setting (Critical Care, In-patient Medicine, Emergency Department, Quality, and/or Case Management experience is helpful).
- Assessment and Communication Skills: Ability to assess patient's clinical status and readiness for discharge, and excellent written and oral communication skills, including strong presentation skills.
- Organizational and Time Management Skills: Excellent organizational skills with the ability to multi-task and prioritize workload in a challenging environment while maintaining mindfulness and accuracy.
- Client Service Orientation: Client service-oriented with the ability to interact and communicate with individuals at all levels (e.g., patients, physicians, hospital staff, hospital administration, and other colleagues).
- Technology Proficiency: Proficiency with technology, including electronic medical systems and Microsoft Office Suite.
- Education and Experience: RN required, MSN preferred, and minimum 5 years nursing experience, with at least 2 years recently in an inpatient acute care or hospital setting.
