Job Title: Family Nurse Practitioner

Community Health Partnership of Illinois is seeking a highly skilled Family Nurse Practitioner to join our team in Mendota, IL.

Job Summary:

The Family Nurse Practitioner will provide primary care to patients of all ages, from infants to elderly individuals, in a variety of healthcare settings, including assigned clinic and local outreach events.

Key Responsibilities:
  • Conduct physical examinations, order and interpret diagnostic tests, diagnose and treat common illnesses and injuries, prescribe medications, manage chronic conditions, provide preventive care and health education, and refer patients to specialists when necessary.
  • Collaborate with other healthcare providers to ensure comprehensive and coordinated care for patients.
  • Participate in quality improvement initiatives, conduct research, and contribute to the development of healthcare policies and practices.
  • Develop and maintain strong communication and interpersonal skills to work closely with patients and their families to provide compassionate and effective healthcare.
Requirements:
  • Master's or doctoral degree in nursing.
  • Licensed as a Registered Nurse.
  • Certification as a Family Nurse Practitioner.
  • Prior experience working in primary care or a related field.
  • Ability to stay current with advances in healthcare and maintain licensure and certification through continuing education and professional development activities.
