Care Transition Liaison Opportunity at LHC Group

Geisinger Home Health, a division of LHC Group, is seeking a skilled Care Transition Liaison to join our team. As a Care Transition Liaison, you will play a vital role in facilitating the transition of patients from hospital or physician's office to home health or hospice care.

Key Responsibilities:
  • Collaborate with physicians, case managers, and other healthcare professionals to identify patient needs and facilitate referrals to LHC Group's home health or hospice services.
  • Communicate with patients and families to educate them on home health and hospice practices and ensure a smooth transition to care.
  • Coordinate start of care between hospitals, physicians, and LHC Group's home health or hospice services.
  • Document activities and productivity using Marketscape CRM and HomeCare HomeBase systems.
Requirements:
  • Prior experience in hospital, physician's office, home health, or hospice settings is preferred.
  • LPN/LVN or licensed medical professional is preferred.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are required.
