Job Summary:
The Registered Nurse Weekend Baylor is a critical role at Aveanna Healthcare, responsible for providing skilled nursing care to patients in their homes. This position requires a dedicated and compassionate individual who can work independently and as part of a team to deliver high-quality patient care.
Key Responsibilities:
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Provide education and support to patients and their families to promote health and well-being.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Coordinate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Document patient progress and communicate effectively with the healthcare team to ensure continuity of care.
Requirements:
- Active RN license in the state of application.
- Valid CPR certification.
Preferred Qualifications:
- Medicare skilled nursing experience.
- Basic understanding of Oasis.
- 1-year RN experience in a healthcare setting.
