Responsibilities:- Provide skilled nursing care to patients in their homes, focusing on promoting health, preventing illness, and alleviating suffering.
- Assess patients' physical, emotional, and social needs, and develop individualized care plans to meet those needs.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Conduct regular patient assessments, update care plans, and communicate with patients and families to ensure continuity of care.
- Document patient progress, including medical history, treatment plans, and outcomes, in accordance with regulatory requirements.
- Participate in quality improvement initiatives, including data collection, analysis, and reporting, to enhance patient care and outcomes.
- Current RN licensure in the state of practice.
- Minimum 1 year of experience in home health nursing, preferably in a travel nursing setting.
- Strong assessment and critical thinking skills, with ability to prioritize tasks and manage time effectively.
- Excellent communication and interpersonal skills, with ability to work collaboratively with patients, families, and healthcare teams.
- Ability to adapt to changing situations and priorities, with flexibility to work in a fast-paced environment.
