The Afterhours AIM RN will report to the Director of AIM.This role involves assessing and coordinating the needs of patients and their families, providing preventive and supportive care to those in palliative situations.
Utilizing assessment, intervention, and educational skills, you will maximize patient comfort and improve quality of life.You will also provide afterhours coverage for designated teams, offering interdisciplinary advanced illness management and support through telephonic, video, and in-person interventions.
Key Responsibilities:
- Triage patient needs via phone until an in-person visit can be arranged.
- Respond urgently to crisis calls from patients and families during afterhours.
- Conduct initial nursing evaluations to determine eligibility for palliative services within 48 hours of referral.
- Identify and reassess the physical, psychosocial, emotional, and environmental needs of patients and families.
- Collaborate with patients, families, attending physicians, and interdisciplinary team members to provide comprehensive care.
- Document problems, goals, interventions, and patient/family responses to palliative care in a timely manner.
- Maintain accurate clinical notes regarding patient conditions.
- Communicate effectively with AIM team members, clinical supervisors, and primary care providers regarding changes in patient status.
- Report any identified needs or issues to supervisors or nurse practitioners.
- Assist in managing medications, durable medical equipment, and supplies in accordance with care plans.
Education:
Associate's or Bachelor's degree in Nursing.
Experience:
Minimum of 2 years as a skilled Registered Nurse, with a preference for experience in palliative care, oncology, hospice, or geriatric care.
Licenses and Certifications:
Current RN license in the state of practice.
CPR Certification.
Skills:
Proficient in Microsoft Office applications and electronic medical record documentation.
Strong communication skills, both written and verbal.
Ability to perform afterhours in-person visits and make sound clinical decisions.
Good problem-solving and time management abilities.
