Job Summary

Alden Estates of Naperville is seeking a skilled LPN Licensed Practical Nurse to join our care team as a Care Team Lead. As a key member of our nursing staff, you will be responsible for providing direct nursing care to our customers and supervising the day-to-day nursing activities performed by our nursing assistants.

Key Responsibilities
  • Customer Care: Provide direct nursing care to our customers, ensuring their physical and emotional needs are met.
  • Nursing Supervision: Supervise and instruct nursing assistants on nursing procedures and protocols, ensuring they are followed in accordance with established policies.
  • Leadership: Lead and direct the nursing team, ensuring they are equipped to provide high-quality care to our customers.
  • Communication: Communicate effectively with customers, family members, physicians, and other healthcare professionals to ensure seamless care coordination.
  • Documentation: Maintain accurate and detailed records of customer care, including progress notes, medication administration, and treatment plans.
  • Medication Management: Administer medications and treatments as ordered by physicians, ensuring accuracy and adherence to stop order policies.
  • Customer Education: Educate customers and their families on disease management, treatment plans, and self-care techniques.
  • Quality Improvement: Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance customer care.
Requirements
  • Licensure: Possess a current, unencumbered license to practice as a LPN in the State of Illinois and/or Wisconsin.
  • CPR Certification: Possess current CPR certification or become certified within two weeks of employment.
  • Knowledge and Skills: Demonstrate knowledge of nursing and medical practices and procedures, as well as excellent communication and leadership skills.
  • Physical Demands: Ability to move intermittently throughout the workday, cope with mental and emotional stress, and perform physical tasks such as lifting and moving equipment and supplies.
