Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Primary Care Provider. As an LPN at ChenMed, you will play a vital role in delivering exceptional patient care and contributing to the success of our rapidly expanding healthcare organization.

Key Responsibilities
  • Provide General Nursing Care: Under the supervision of physicians and clinicians, you will provide general nursing care to patients, ensuring their health and well-being are prioritized.
  • Assist with Exams and Procedures: You will assist with exams, procedures, patient education, and maintenance of medical records, ensuring accurate and timely documentation.
  • Collect and Record Patient Information: You will compile patient health information, take and record vital statistics, and conduct basic care treatments, such as taking blood pressure and collecting lab specimens.
  • Prepare Patients for Examination: You will prepare patients for examination, assist physicians with exams, and explain treatment procedures, medications, diet, and physicians' instructions to patients.
  • Administer Medications and Treatments: You will prepare and administer medications ordered by physicians, fill pill boxes as ordered, change dressings, apply bandages, remove sutures, and perform first aid procedures.
  • Screen Calls and Walk-ins: You will screen calls and walk-ins, collect pertinent clinical information, relay it to physicians, implement orders as appropriate, and document observations and interventions in the medical record.
  • Maintain Supplies and Equipment: You will maintain supplies and equipment, stock and sterilize instruments, practice OSHA safety standards, and perform accurate, legal, and ethical documentation at all times.
  • Communicate Effectively: You will use effective communication skills with appropriate medical terminology, follow legal and ethical professional conduct, and complete internal certification to use the ChenMed Rx system to support physician medication dispensing.
  • Perform Other Duties: You will perform other duties as assigned and modified at manager's discretion, ensuring the success of our healthcare organization.
Requirements
  • Education and Experience: High school diploma or equivalent required; graduation from a nationally accredited school for practical or vocational nursing required; current, active LPN license to practice in the state of employment required; a minimum of 1 year of work experience as an LPN required; a minimum of 3 years overall clinical experience preferred.
  • Skills and Abilities: Excellent clinical skills to identify, diagnose, and resolve patient issues; good non-clinical skills to assist with dressing, physical exercise, restroom visits, etc.; good motor skills to effectively calibrate equipment, manipulate intravenous equipment, administer injections, position, and transport patients when needed; excellent oral and written communication skills; ability to effectively collaborate with team members; mindset focused on solving problems for patients and achieving team goals; skill in operating phones, personal computer, software, and other IT systems; ability to maintain effective and organized systems to ensure timely patient flow; ability to exercise patience, compassion, and empathy for patients and family members; ability to act calmly in busy or stressful situations; good time management to ensure tasks are completed timely and efficiently.
