Residential Nurse

At Phaxis, we are seeking a highly skilled and compassionate Residential Nurse to join our team. As a Residential Nurse, you will be responsible for providing high-quality nursing care to individuals with intellectual and developmental disabilities.

• Ensure medical care for individuals within a designated cluster, including identifying and developing individualized plans for nursing services.
• Monitor and review medical reports, visit forms, diagnostic tests, and laboratory results to ensure timely and effective care.
• Coordinate medical appointments, including tracking and reviewing health status, and ensuring prompt rescheduling of cancelled appointments.
• Provide initial and ongoing supervision, training, and support to unlicensed direct care staff on individual's chronic medical conditions and nursing tasks.
• Conduct annual clinical performance evaluations for unlicensed direct care staff, including procedures such as medication administration.
• Monitor and visit individuals who are hospitalized for medical or psychiatric reasons, coordinating communication between the hospital and the Habilitation Team.
• Participate in a 24-hour, 7-day on-call system, providing direction and support to residential staff, and ensuring back-up nursing coverage during use of accrued agency time.

• NY State licensed Registered Nurse required; current CPR certification required.
• One year experience working with ID/DD population is preferred.
• Ability to drive is necessary to perform job responsibilities.
