Job Title: Social Worker for Population Health

Job Summary:

The Social Worker for Population Health will work closely with our multidisciplinary team to provide case management services to patients in their home setting. This includes reviewing and monitoring patient engagement, providing education and support, and collaborating with healthcare providers to ensure comprehensive care.

Responsibilities:
  • Provide case management services to patients in their home setting, including reviewing and monitoring patient engagement
  • Collaborate with healthcare providers to ensure comprehensive care and address patient needs
  • Provide education and support to patients and families to promote health and well-being
  • Use professional judgment to assess, plan, and evaluate patient needs and develop effective care plans
  • Act as a consultant to staff for social work service needs
Requirements:
  • Education: Bachelor's degree in Social Work required, Master's degree in Social Work preferred
  • Licensure/Certification: Licensed Social Worker by the State of Ohio Counselor and Social Worker Board
  • Experience: Minimum of two years of experience in social work, one year in a medical setting
  • Effective communication and interpersonal skills
  • Current knowledge of legal, regulatory, and accrediting requirements and standards related to the practice of social work
