Job Summary

We are seeking a skilled Occupational Therapist to join our team at Providence Humboldt Home Health. As a Home Health Occupational Therapist, you will work with patients in their homes to help them recover from illness or injury and maintain their independence.

Responsibilities
  • Conduct thorough assessments of patients' physical and cognitive abilities to develop personalized treatment plans.
  • Implement and modify treatment plans as needed to ensure optimal patient outcomes.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide education and training to patients and caregivers on proper techniques and strategies for daily living and mobility.
  • Document patient progress and outcomes in accordance with regulatory requirements.
Requirements
  • 1 year of experience in skilled home health as an Occupational Therapist.
  • BCLS certification required.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Strong problem-solving and critical thinking skills.
