Key Responsibilities- Develop and implement individualized service plans for residents
- Provide nursing care according to established policies and procedures
- Communicate with physicians, family members, and other healthcare professionals as needed
- Supervise and oversee the care delivered by the Home Support team
- Participate in care conferences and other resident-related meetings
- Graduate of an accredited Registered Nurse degree program
- Current Registered Nurse license for the state in which they practice
- Recent home care experience preferred
