Home Health Nurse Opportunity in NH

BAYADA Home Health Care is seeking a skilled Registered Nurse to provide high-quality care to patients in their homes in Hooksett, New Hampshire.

This is an excellent opportunity for a dedicated nurse to make a meaningful difference in the lives of clients and their families. As a BAYADA Home Health Nurse, you will have the chance to work independently, prioritize your caseload, and provide one-on-one care to patients in need.

Responsibilities:

  • Develop and implement personalized care plans tailored to each client's unique needs and goals.
  • Conduct thorough assessments, monitor client progress, and report changes to the Clinical or Client Services Manager.
  • Provide direct patient care, including medication administration, wound care, treatments, and procedures.
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination.
  • Document observations, interventions, and evaluations accurately and thoroughly.

Qualifications:

  • A current license as a Registered Nurse in New Hampshire.
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
  • Prior home care experience is strongly preferred but not required.
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
  • Ability to work independently, manage time effectively, and prioritize tasks.
  • Strong interpersonal skills, with the ability to communicate effectively with diverse clients and families.
