Job Summary:

We are looking for a dedicated and detail-oriented Registered Nurse to work in our Hospital Care at Home Program. The successful candidate will be responsible for providing clinical monitoring and care for patients, including assessing psychosocial needs, recommending services, and creating collaborative care plans. Our team of experts will treat and monitor patient health with the highest level of care, utilizing a combination of in-person, virtual visits, and monitoring through our Telehealth Platform.

Responsibilities:
  • Provide clinical monitoring and care for patients in their homes, utilizing a range of technology tools and software.
  • Assess psychosocial needs and recommend services for patients.
  • Create collaborative care plans with patients and their care teams.
  • Perform case management functions and escalate clinical concerns to providers as needed.

Requirements:

To be successful in this role, you will need:

  • A current RN license in the state of Delaware.
  • Experience working in a home health care setting or a similar environment.
  • Strong clinical skills and attention to detail.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
