Nurse Care Specialist

Job Summary:
An Admission Registered Nurse is the primary RN responsible for delivering high-quality, patient-centered care to individuals in their place of residence. This role involves completing initial assessments, administering skilled nursing care, and collaborating with healthcare teams to develop and implement personalized care plans.
Key Responsibilities:
- Provides and coordinates services in accordance with the patient's plan of care.
- Makes initial evaluations and regularly reevaluates the patient's nursing needs.
- Initiates and revises the plan of care as necessary.
- Delivers skilled nursing services requiring specialized skills.
- Initiates preventive and rehabilitative nursing procedures.
- Prepares clinical and progress notes for each patient visit and summaries of care conferences.
- Notifies healthcare teams of changes in the patient's condition and needs.
- Counsels patients and their families on meeting nursing and related needs.
- Participates in in-service programs and stays up-to-date with industry developments.
- Understands and adheres to established Agency policies and procedures.
- Processes orders and notifies physicians of patient needs and changes in condition.
- Refers patients to specialized healthcare professionals as needed.
- Supervises and educates other nursing personnel.
- Conducts patient care conferences and participates in peer review and quality assessment.
- Takes on-call duty as assigned.
- Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with Agency-defined time frames.
Requirements:
- Current nursing license in good standing with the Board in the state of practice.
- Preferred one (1) year of experience as a nurse.
