We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality care to patients in their homes. As an LPN, you will play a vital role in supporting our patients and their families, ensuring they receive the best possible care and experience.
Responsibilities- Provide direct care to patients in accordance with a comprehensive care plan in the home care setting.
- Document patient services accurately and in a timely manner.
- Assess and educate patients and their families/caregivers on health-related topics.
- Participate in the coordination of home care services and quality assessment activities.
- Ensure compliance with all state and federal legal and regulatory requirements.
- Graduate of an accredited practical or vocational nursing program with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three or more years preferred.
- Experience in community health, hospice, or med/surg is preferred.
- Commitment to clinical and documentation excellence.
