Patient Care ResponsibilitiesIn collaboration with patients and their families, the Rehabilitation Specialist is accountable for the safe and effective delivery of care within their scope of practice. This encompasses:
- Conducting thorough examinations and evaluations
- Diagnosing and planning appropriate interventions
- Establishing measurable outcomes
Full-time employment is available, with authorization to work 80 hours per pay period.

M Health Fairview Rehabilitation provides a wide array of services across 10 acute-care hospitals, three post-acute settings, and 70 outpatient therapy clinics, serving both adult and pediatric populations.
Our team consists of Physical, Occupational, and Speech Therapists, as well as Audiology and Cardiac & Pulmonary Rehabilitation specialists. We emphasize collaboration across all medical settings and offer numerous specialty programs.
As an academic health system, we are dedicated to fostering a culture of learning, with a strong focus on evidence-based, patient-centered care. Our continuing education program provides over 40 courses annually at no cost to employees, along with support for external continuing education opportunities.
As a member of the M Health Fairview rehabilitation team, you will have access to formal mentorship, clinical specialization, and recognition through our clinical specialist program.
Role ResponsibilitiesThe Rehabilitation Specialist will:
- Partner with patients and families for evaluations and re-evaluations
- Develop individualized care plans through coordination
- Analyze data to identify needs and potential risks
- Implement and modify care plans based on patient responses
- Coordinate care with interdisciplinary teams and establish discharge plans
- Provide skilled interventions and education to patients and caregivers
Documentation and Professional Standards
Maintain accurate documentation in accordance with department standards and billing requirements. Prioritize tasks effectively and adhere to professional licensure and certification standards.
Quality ImprovementEngage in quality improvement initiatives and support entity/system goals.
QualificationsRequired:
- Physical Therapy registration/licensure in Minnesota or a temporary license
- Current Basic Life Support (BLS) certification
Preferred:
- Doctorate degree in Physical Therapy
- At least one year of experience in Physical Therapy practice
