About the RoleWe are seeking a qualified Physical Therapist to provide physical therapy services to adult patients in the home, following physician's orders. As a Physical Therapist - Home Visits, you will be responsible for evaluating patients, developing treatment plans, and providing instruction and education to patients and their caregivers.
Responsibilities- Evaluate patients using diagnostic muscle, nerve, joint, and functional ability tests and measures.
- Develop a written problem list and plan of care, including types and frequencies of treatment.
- Document clinical notes on the date of the visit into medical records and keep all records and summaries up-to-date.
- Treat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence.
- Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment.
- Graduate of an accredited Physical Therapy program.
- Current, valid Physical Therapy license issued by NYS Department of Education.
- One year of practical experience as a licensed physical therapist.
The Physical Therapist - Home Visits will be primarily in a non-office setting and may be exposed to outdoor conditions. The working conditions are classified as medium work, exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
