Key Responsibilities - Primary Care or Mental Health
- Conduct rounds at designated facilities, addressing the acute, subacute, and primary care needs of this vulnerable population.
- Collaborate closely with the OnSite team, nursing home personnel, residents, and their families.
- Proactively monitor patient progress and evaluate the effectiveness of recommended treatments.
- Educate patients on best practices for health management.
- Incorporate various treatment modalities, prioritizing non-pharmacological approaches whenever feasible.
- Hold a Master of Science in Nursing with certifications such as FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP.
- Possess an active nursing license in South Carolina.
- Have DEA registration.
- Exhibit excellent communication and documentation skills.
- Demonstrate strong patient assessment capabilities.
- Maintain a strong, ambitious, and patient-centered work ethic.
