About the Role

PennState Health is seeking a skilled Social Worker to join our team in Hershey, Pennsylvania. As a Social Worker I, you will be part of an interdisciplinary team responsible for maintaining continuity of patient care for identified patient populations from admission to discharge.

Key Responsibilities
  • Utilize appropriate criteria for inpatient care and plan timely discharges.
  • Provide emotional support to patients and their support systems in managing complex medical conditions in an inpatient setting.
  • Conduct crisis intervention, individual, family, group, bereavement, and end-of-life counseling.
  • Participate in designing and implementing institutional programs/initiatives to improve patient access to medical center resources, quality of care, cost reduction efforts, and patient satisfaction.
Requirements
  • Masters of Social Work degree from a Council of Social Work Education school of social work.
  • Minimum of two (2) years of acute care case management/social work experience.
  • Knowledge of PC-based computer software (Cerner, Navihealth/Curaspan).
  • Current social work licensure in Pennsylvania.
  • Certification in case management from a nationally recognized organization.
