Key Responsibilities:
- Conduct thorough assessments and document findings in a clear and compliant manner, adhering to CMS regulations.
- Ensure all diagnoses are accurately coded using ICD10 standards and documented to support the visit's diagnosis.
- Address all quality elements and ensure proper documentation.
- Perform an initial review of medications.
- Complete all mandatory training and educational requirements.
- Travel between care sites as required.
- Care Coordination:
- Communicate with patients and relevant team members regarding any medical concerns during your visit.
- Expected Behaviors for Program Enhancement:
- Demonstrate creativity and original thinking in developing improved methods for addressing concerns.
- Work independently with minimal supervision.
- Input available hours into a web-based application at least one month in advance.
- Show initiative in achieving individual and team goals.
- Participate in quality initiatives.
- Regularly check Optum email for training, communications, and scheduling updates.
Required Qualifications:
- For NPs: Certified Nurse Practitioner through a national board.
- Graduate of an accredited MSN program and board certified through AANP or ANCC.
- For PAs: Certified Physician Assistant through a national board; graduate of an accredited PA program and currently board certified by NCCPA.
- Active and unrestricted license in your state of residence.
- Current DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
- Reliable transportation for travel to client and patient sites.
- Ability to lift a 30-pound bag and navigate various dwelling conditions.
- Ability to establish a collaborative practice agreement, if applicable.
- 1+ years of post-graduate experience in Long Term Care.
- Knowledge of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end-of-life discussions.
- Proficient in computer skills, including electronic medical records documentation.
- Ability to foster positive customer relationships.
- Adaptability to change.
