Position Overview:
· Role: Deliver comprehensive, patient-focused care to individuals facing significant health challenges, often dealing with multiple chronic illnesses, ensuring they receive support when and where it is most needed. This model of continuous care offers clinical, economic, and emotional advantages.
· Vision: Patient Care Medical Group is dedicated to understanding the holistic needs of each patient—physical, emotional, and social—to provide tailored care through collaboration with a multidisciplinary team, aiming for the highest possible outcomes.
· Approach: Ensure timely interventions to manage chronic conditions effectively, preventing unnecessary emergency visits and hospital admissions, while addressing social factors that impact health and providing compassionate support to patients.
· Target Areas: Focus on Acute Discharges, Skilled Nursing Facilities, Community/Home Care Patients, and Assisted Living Facilities.
Key Responsibilities:
- Conduct gap care visits in skilled facilities, treating patients when PCMG is not the primary care provider, and collaborating with attending physicians as needed.
- Perform facility rounds and conduct thorough patient examinations.
- Provide primary care services to patients in home settings or assisted living facilities.
- Evaluate abnormal vital signs and lab results, making recommendations or notifying the attending physician as necessary.
- Review new admissions and their corresponding orders.
- Participate in meetings to review readmissions, summarizing all acute discharges and collaborating with the interdisciplinary team.
- Independently conduct comprehensive history and physical assessments for complex patients.
- Develop and manage individualized patient treatment plans.
- Order and interpret diagnostic and therapeutic tests based on the patient's specific needs.
- Prescribe suitable pharmacological and non-pharmacological treatments.
- Implement interventions to help patients achieve or maintain physiological stability, including assisting in surgical roles or collaborating with primary care providers.
- Monitor the effectiveness of treatment interventions.
- Facilitate patient transitions across various healthcare settings, including admissions, transfers, and discharges.
- Collaborate with team members to make appropriate referrals.
- Support decision-making for staff, patients, and families by providing educational resources.
Qualifications:
- Completion of a PA Degree from an accredited institution and a minimum of two years of relevant experience or training.
Physical Demands:
- Ability to stand, walk, and use hands for various tasks; capable of lifting weights up to 50 pounds.
