Job Summary

Community Based Care is seeking a highly motivated and experienced ICF Program Coordinator to join our team. As an ICF Program Coordinator, you will play a key role in assisting first-line supervision in the facility and being responsible for the staff, maintaining the program in accordance with standards.

Key Responsibilities
  • Assist in the development and implementation of the goals and objectives listed in each program member's Individualized Personal Plan (IPP).
  • Provide staff coverage when needed and communicate with the Regional Program Manager and/or Program Manager regarding concerns, problems, or questions regarding the program's member's objectives and plans.
  • Develop, coordinate, and facilitate Inservice training for program staff when directed, in collaboration with the Program Manager.
  • Ensure that staff follows a program schedule, including members' Active Treatment modality, IPP schedule, Program Community Integration schedule, and medication administration schedule.
  • Assist in completing new hires Day One training and annual training, in collaboration with the Regional Manager and Program Manager.
  • Perform data entry for the program by entering personal information into electronic record keeping and training programs.
  • Maintain the facility in a sanitary condition and coordinate repairs with supervision.
  • Ensure that members' Activities of Daily Living (ADLs), including bathing, toileting, etc., are completed based on goals documented in the member's IPP.
  • Issue progressive disciplinary action, including coaching and supervision when needed.
  • Coordinate the replacing of supplies and equipment.
  • Ensure the program follows statutory, certification, and licensure regulations/standards.
  • Comply with all corporate policies and procedures, HIPAA guidelines, DHHS rules, and Medicaid state and federal guidelines.
Additional Responsibilities
  • Maintain adherence to authorized budgets.
  • Ensure the delivery of quality services to the individuals in the program.
  • Perform other duties as required.
Qualifications
  • High school diploma or equivalent.
  • Demonstrates ability to read, write, understand, and follow directions.
  • Demonstrated ability to use computers, Google Drive, and other technology systems.
  • One year of experience working in the human services field.
  • Willingness to receive COVID-19 vaccination.
Skills/Experience
  • Assist in the recruitment of program staff.
  • Responsible for the provision of staff training.
  • Provide input in policy/procedure formulation and implementation.
  • Requires experience in direct care services or related fields.
