Position Overview:
Under the guidance of the Thoracic Program Leader, the Lung Nodule Nurse Navigator is responsible for ensuring that cancer patients are effectively guided through the healthcare system, receiving appropriate medical, support, and educational services throughout their journey. This role involves establishing, nurturing, and maintaining strategic relationships with community physicians and their office personnel.
Your Contributions to the Team:
- This position focuses on managing and following up on incidental lung nodule referrals, particularly high-risk nodules identified through the Epic dashboard and lung cancer screenings.
- Facilitates continuity of care for patients across hospital departments and provides follow-up for laboratory, radiology, and pathology procedures. Timely identification and resolution of issues are handled effectively to ensure optimal patient care and physician relations.
- Coordinates and participates in weekly thoracic tumor board meetings and lung nodule clinics, tracking scheduled procedures, following up on pathology results, and collaborating with pathology/radiology to monitor outcomes.
- Works closely with referring providers to coordinate additional evaluations for patients based on treatment planning, including diagnostic procedures and referrals.
- Understands and adheres to the Oncology Lung Navigator Workflow Standard Operating Procedure.
Qualifications for Success:
Minimum Requirements:
- Graduate of an accredited nursing program.
- Current valid State of Florida Registered Nurse license
- Valid Driver's license
- Basic Life Support (BLS) certification
- At least one year of experience in Oncology and/or thoracic nursing
Preferred Qualifications:
- Bachelor’s degree (BSN) or Master’s degree (MSN) in Nursing
- Current certification in Oncology nursing
