HopeHealth, Inc. is seeking a highly skilled Patient Care Specialist to join our medical team. As a key member of our internal referral process, this individual will coordinate and facilitate effective communication between departments.

Key Qualifications:

  • Able to work in a fast-paced environment with multiple priorities

Essential Job Functions:

  1. Navigate incoming referrals to departmental providers
  2. Schedule new patients with the appropriate medical provider and ensure the receipt of medical records
  3. Schedule outgoing referrals for MRI and other required medical screenings
  4. Maintain up-to-date documentation through timely scanning and filing
  5. Patient Check-in/Check-out and demographics verifications are completed as necessary
  6. Answer patient phone calls and assist with follow-up calls to ensure compliance with patient care plans
  7. Communicate effectively with staff through documentation as assigned
  8. Complete all records and reports as required
  9. Participate in staff meetings and in-service training as needed
