Key Responsibilities- Conduct thorough assessments of patients and their families to identify social, emotional, and environmental factors that may impact their care and well-being.
- Develop and implement individualized plans to address the unique needs of each patient, including connecting them with community resources and support services.
- Collaborate with healthcare providers, social workers, and other stakeholders to ensure seamless care coordination and transitions.
- Provide counseling and support to patients and families experiencing crisis situations, such as injury or death.
- Participate in quality improvement initiatives to enhance the overall quality of care and services provided by the Social Services department.
- Stay up-to-date with industry developments and best practices through ongoing education and professional development.
- Bachelor of Social Work degree from an accredited institution (Master's degree preferred).
- Current licensure as a Licensed Professional Counselor (LPCC), Licensed Professional Counselor (LPC), Licensed Mental Health Counselor (LMHC), Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or equivalent.
- Minimum of [X] years of experience in counseling or social work, preferably in a healthcare setting.
- Must be able to work in a fast-paced environment with multiple priorities and deadlines.
- Ability to maintain confidentiality and handle sensitive information with discretion.
