Job SummaryWe are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Las Cruces, NM. As an LPN with Ambercare Homecare, you will play a vital role in providing direct patient care and implementing care plans initiated by registered nurses.
Responsibilities- Direct Patient Care: Provide high-quality patient care as defined in the state Nurse Practice Act, ensuring the health and well-being of our clients.
- Plan Implementation: Implement care plans initiated by registered nurses, working collaboratively to achieve optimal patient outcomes.
- Documentation: Maintain accurate and timely documentation, consistent with the plan of care, to ensure seamless communication among healthcare professionals.
- Patient Education: Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
- Interdisciplinary Collaboration: Participate in coordination of home health services, reporting identified needs for other disciplines to the registered nurse and/or Clinical Supervisor.
- Equipment and Supplies: Use and/or prepare equipment and supplies effectively and efficiently, adhering to aseptic technique.
- Personal and Professional Growth: Participate in personal and professional growth and development, staying up-to-date with industry best practices and advancements.
- Additional Duties: Perform other duties as assigned by the registered nurse, ensuring the delivery of exceptional patient care.
- Education: Graduate of an accredited practical nurse or vocational nursing program.
- Experience: Three (3) years nursing experience, with community/home health or medical/surgical experience preferred.
- Licensure: Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
- Certification: Possess and maintain valid CPR certification.
- Transportation: Must have reliable transportation, a current driver's license, and appropriate automobile insurance.
