Job Summary

We are seeking a skilled Case Manager to join our team at The Salvation Army. As a Case Manager, you will provide comprehensive support and expertise to clients, assessing their needs and developing individualized service plans to enhance their quality of life.

Key Responsibilities
  • Conduct intake assessments with new clients to identify their housing, health, financial, and psychological needs.
  • Develop and implement individualized service plans to meet client needs, including referrals to community resources and assistance with accessing necessary services.
  • Collaborate with clients, families, and other stakeholders to ensure effective case management and coordination of services.
  • Provide counseling and support to clients on housing, health benefits, financial support, and available community services.
  • Maintain accurate documentation of client progress and outcomes, including case notes and statistical reports.
Requirements
  • Bachelor's degree in Social Work or related field preferred.
  • Minimum of one year of experience working with low-income and/or homeless individuals and families.
  • Valid Class C California Driver's License required.
  • Ability to pass a criminal background check.
Preferred Qualifications
  • Bilingual English/Spanish preferred.
  • Proficient in Microsoft Windows and use of Word and Excel.
  • Ability to teach life skills classes.
