Job Summary

We are seeking a highly skilled Case Manager to join our team at PAM Health Specialty Hospital of San Antonio. As a key member of our interdisciplinary team, you will be responsible for coordinating care for patients, ensuring efficient utilization of resources, and determining the appropriate level of care.

Responsibilities:
  • Coordinate patient care across various departments, ensuring seamless transitions and optimal outcomes.
  • Conduct thorough reviews of admission records to identify areas for improvement and optimize resource allocation.
  • Develop and implement discharge plans tailored to individual patient needs, promoting timely and effective transitions.
  • Collaborate with the interdisciplinary team to achieve desired financial and quality outcomes, enhancing our hospital's reputation and services.
Requirements

To excel in this role, you should possess:

  • A degree in a relevant field (e.g., occupational therapy, physical therapy, nursing, respiratory therapy, or social work) from an accredited institution.
  • Current licensure in the state where the hospital resides.
  • At least three years of experience in an acute, IRF, or LTACH setting, preferably with a focus on inpatient care.
  • Ongoing certification in Basic Life Support (BLS).
Compensation

The estimated salary for this position is $85,000 per year, reflecting the requirements and responsibilities associated with this role. This figure may vary based on factors such as location, experience, and qualifications.
