Job Summary:

To provide compassionate spiritual support and counseling to home care patients, families, and staff in a culturally inclusive and sensitive manner. As a member of the interdisciplinary patient care team, the Chaplain will participate in patient care planning, document interventions and outcomes, and adhere to regulatory guidelines.

Key Responsibilities:
  • Administrative Duties:
    • Participate in quality assurance activities, including committees for patient satisfaction scores, quality improvement projects, and other initiatives as requested or in collaboration with agency leadership.
    • May serve on committees and work in collaboration with committees such as bioethics, employee assistance programs, and other related groups.
    • Conduct in-services and continuing education programs to improve the integrated model of care, including new employee orientation and interdisciplinary meetings.
    • Provide staff support to maintain spiritual resilience in healthcare, including individual development groups, all-staff reflections, speakers, and rituals.
    • Provide faith- and value-based rites and rituals and maintain collaboration with community clergy to ensure support for staff, patients, and families with religious-specific spiritual care needs.
    • Act as a consultant to community liaisons and faith leaders regarding serving in a healthcare setting as appropriate for collaboration in meeting home care patient needs.
    • Provide spiritual support and counseling to staff members, including debriefs, complex grief, and other requested support.
    • Maintain compliance with the standards of practice for professional chaplains and demonstrate common qualifications and competencies for professional chaplaincy.
    • Attend education programs to improve skills related to provision of home care services.
    • Other duties as assigned.
  • Direct Patient Care:
    • Provide spiritual support and counseling to home care patients, families, and staff in a culturally competent manner that upholds the Kaiser Permanente principles of responsibility.
    • Clinical chaplains are members of the interdisciplinary patient care team, using evidence-based outcome-oriented assessment tools to participate in creating patient care plans that include acuity mapping to define frequency of visits/duration, document interventions/outcomes, and meet regulatory guidelines.
    • Provide spiritual support or visits at the time of death as assessed by patient/family need.
    • Support the admission/consent process in collaboration with the interdisciplinary team in explaining hospice benefits to patients and families as requested by leadership.
    • Support bereavement care plans for surviving family members for up to 13 months after patient death, including phone support, in-home visitations, bereavement support groups, and memorial services.
    • Assist with funeral services for deceased patients when requested and as scheduling allows.
    • Vary schedule in collaboration with leadership to accommodate needs for 24-hour coverage.
Requirements:
  • Experience:
    • Minimum of one (1) year of experience as a clinical chaplain/spiritual counselor in a hospital, ambulatory, home health, palliative care, or hospice setting required.
  • Education:
    • Masters degree in divinity, theology, or related field from an accredited seminary, college, or university required.
  • License, Certification, Registration:
    • Driver's license (California)
    • Basic life support required at hire
  • Additional Requirements:
    • Must be able to serve and support people of all religions, worldviews, and spiritual perspectives.
    • Maintains adequate automobile liability insurance as required by hospice program policy.
    • Meets medical center/home care requirements for health appraisal and annual PPD testing.
    • Behaves in a courteous, professional manner with patients, staff, and healthcare professionals and maintains patient confidentiality.
    • Appropriate age-specific competencies to be successfully completed by end of orientation: infant, toddler, child, adolescent, adult, and geriatric.
    • Conveys a sense of maturity and sensitivity with good listening skills as evident on interviewing.
    • Ability to prioritize and manage multiple tasks and crisis situations.
    • Demonstrated excellent reading, writing, and verbal communication skills.
    • Ability to recognize urgent patient needs and how to report it.
    • Must be able to work in a labor/management partnership environment.
  • Preferred Qualifications:
    • Two (2) or more years of experience as a clinical chaplain/spiritual counselor in a hospital, home health, palliative, or hospice setting.
    • Board-certified chaplain certification from the Association of Professional Chaplains.
    • Minimum of four (4) units of clinical pastoral education from an ACPE-accredited program.
    • PhD, DMin, ThD, or EdD from an accredited seminary, college, or university. Certificates specializing in palliative or hospice care.
