About the Role

We are seeking a highly skilled Physical Therapist Assistant to join our Skilled Home Care division at Interim HealthCare Home Health and Hospice. As a Physical Therapist Assistant, you will play a vital role in providing high-quality patient care and contributing to the success of our team.

Key Responsibilities
  • Participate in ongoing interdisciplinary assessments of patients to ensure comprehensive care.
  • Act at the direction of the treating physical therapist and in accordance with the plan of care to provide exceptional patient outcomes.
  • Collaborate with patients, representatives, and caregivers to develop and evaluate the plan of care, ensuring patient-centered care.
  • Provide services ordered by physicians as indicated in the plan of care, promoting patient progress and well-being.
  • Assist the treating physical therapist in documenting patient progress and response to treatment, ensuring accurate and timely documentation.
  • Counsel and educate patients and their families, as well as other caregivers, to promote patient progress and meet healthcare-related needs.
  • Effectively communicate with the interdisciplinary healthcare team to ensure coordination of patient care and planning for discharge.
  • Communicate with physicians and other healthcare practitioners involved in the plan of care to ensure seamless care coordination.
  • Access and utilize personal health information necessary to fulfill duties and responsibilities, adhering to organizational and departmental guidelines.
Requirements
  • Current licensure as a physical therapist assistant in the state of Michigan.
  • At least two years of experience as a physical therapist assistant in the last five years.
  • Active CPR certification.
  • Previous home health experience is highly desirable.
