Job Summary:

The Registered Nurse (RN)/Case Manager plays a vital role in providing high-quality home health care services to patients in need. This position requires a skilled and compassionate individual who can assess patient needs, develop and implement care plans, and provide education and support to patients and their families.

Key Responsibilities:
  • Patient Care:
    • Conduct thorough assessments of patient conditions and needs at each home visit
    • Develop and implement individualized care plans that address patient goals and outcomes
    • Provide education and training to patients and their families on proper home health care procedures and strategies
    • Administer medications and treatments as prescribed by physicians
    • Monitor patient progress and make adjustments to care plans as needed
  • Communication:
    • Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care
    • Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans
    • Document patient progress and communicate with the healthcare team as needed
Requirements:
  • Graduate of an accredited school of nursing
  • Current licensure in the state of Indiana and current CPR certification
  • Excellent observation, verbal, and written communication skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
  • Ability to lift, position, or transfer patients as needed
