Key Responsibilities- Utilize technological tools to manage patient populations and identify high-risk patients requiring care coordination.
- Conduct comprehensive clinical assessments, including disease-specific, age-specific, medical, behavioral, pharmacy, social, and end-of-life needs, to inform care coordination.
- Work collaboratively with the interdisciplinary team to develop goals and plan interventions to maximize patient outcomes.
- Monitor patient compliance with care plans and perform reassessments to update plans as needed.
- Ensure care gaps are closed around specialty disease, chronic disease, and surgical episodes.
- Serve as the primary patient contact for the team related to condition or surgical episode and facilitate access to services.
- Coordinate members of the patient care team and serve as a liaison between patients, families, and physicians.
- Assist in managing transitions of care across care settings, ensuring optimal communication and planning.
- Graduate from an accredited school of professional nursing.
- Current state licensure as a Registered Nurse (RN).
- Basic Life Support (BLS) certification through the American Heart Association.
- Three to five years of nursing experience.
