Clinical Care Specialist

The Clinical Care Specialist is responsible for delivering high-quality patient care in accordance with developed care plans and physicians' orders. This role requires strong clinical skills, effective communication, and a commitment to maintaining professional boundaries.
Key Responsibilities:
• Develop and implement individualized care plans through collaboration with healthcare teams and client input.
• Educate clients and their family members on specific needs and provide support throughout the care process.
• Properly orient and train primary caregivers to ensure optimal functioning levels for each client.
• Coordinate hands-on client care through timely documentation and communication with healthcare providers.
Requirements:
• Active RN License in the state of application
• Valid CPR certification
• Preferred: Medicare Skilled Nursing experience and basic understanding of Oasis
