Responsibilities- Provide comprehensive nursing care to patients in an ambulatory setting, including assessment, planning, implementation, and evaluation of care.
- Collaborate with primary care providers and support staff to ensure seamless care coordination and effective communication.
- Develop and implement individualized care plans that address the unique needs and preferences of each patient.
- Provide education and support to patients and their families on health promotion, disease prevention, and management of chronic conditions.
- Document all patient interactions and care provided in the electronic medical record, ensuring accurate and timely documentation.
- Participate in quality improvement initiatives and contribute to the development of clinical care pathways and protocols.
- Stay up-to-date with the latest evidence-based practices and guidelines in ambulatory care nursing.
- Graduate of an accredited school of nursing with a current RN licensure in the state of employment.
- 2+ years of experience as a Nurse in a primary care or ambulatory setting, with a strong background in patient education and care coordination.
- AHA BLS certification and ability to maintain current certifications.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
