• Conduct initial and ongoing clinical evaluations to assess patient needs and functional levels, including OASIS assessments at designated intervals.
• Collaborate with physicians and care-team members to develop and update a comprehensive Plan of Care.
• Provide therapeutic interventions and assess equipment requirements as necessary.
• Evaluate care outcomes and engage in effective discharge planning.
• Implement infection control measures and standard precautions while coordinating and delivering patient care to prevent disease transmission.
• Accept clinical assignments aligned with your education and competencies.
• Supervise Physical Therapy Assistants (PTA) and Home Health Aides (HHA) as appropriate.
• Complete all necessary documentation promptly.
• Fulfill mandatory continuing education requirements set by the agency and licensing board, and participate in in-service training programs.
• Utilize effective communication skills to consult with families and other agency personnel as needed.
• Uphold the agency's philosophy and administrative policies.
• Demonstrate commitment to professional development and competency.
• Proficiently use technology to facilitate communication and perform essential job functions.
Qualifications:
Education:
• Bachelor of Science in Physical Therapy or higher from an accredited program recognized by the American Physical Therapy Association.
Licensure:
• Current Texas State License as a Physical Therapist and a valid Texas Driver's License.
Skills:
• Proficient therapy skills in accordance with accepted standards of practice. Strong interpersonal skills. Current BLS certification for healthcare providers. Familiarity with durable medical equipment.
Transportation:
• Reliable transportation and valid auto liability insurance.
Experience:
• Preferred experience in Home Health and manual therapy; knowledge of HomeCare HomeBase is a plus.
