Job Summary:The LPN will work under the direction of a Registered Nurse (RN) to deliver efficient and effective nursing care, achieving positive clinical outcomes and patient/family satisfaction. The ideal candidate will operate within the scope of practice defined by the State Nurse Practice Act, contributing to nursing assessments and care planning, providing direct patient care, and supervising care provided by unlicensed staff.
Responsibilities:- Patient Evaluation: Conduct thorough assessments of patients to identify their needs and develop individualized care plans.
- Care Planning: Collaborate with the RN to develop and implement care plans that meet the unique needs of each patient.
- Provision of Direct Patient Care: Provide high-quality nursing care to patients, including administering medications, performing treatments, and monitoring patient conditions.
- Monitors patient care provided by unlicensed staff: Supervise and evaluate the care provided by unlicensed staff to ensure it meets the standards of care.
- Uses Point Click Care (PCC) according to the Business Process: Utilize the PCC system to document patient information, track progress, and communicate with the healthcare team.
- Performs drug count when beginning and ending shift and whenever necessary: Conduct regular drug counts to ensure the accuracy and security of medications.
- Assists physician in rounds and examination of patients as needed: Collaborate with physicians to provide high-quality patient care and support.
- Participates in shift-to-shift communication between incoming and outgoing nursing staff: Ensure seamless communication between nursing staff to maintain continuity of care.
- Collaborates and coordinates with other departments to provide timely effective care consistent with individual's needs, choices, and preferences: Work collaboratively with other departments to provide comprehensive care that meets the unique needs of each patient.
- Contributes to an environment that is respectful, team-oriented, and responsive to the concerns of staff, patients and families: Foster a positive and supportive work environment that promotes respect, teamwork, and open communication.
- Enhances nursing practice by attending all mandated in-service programs: Stay up-to-date with the latest nursing practices and protocols by attending required in-service programs.
- Assists with the orientation of newly hired nursing staff: Provide guidance and support to new nursing staff to ensure a smooth transition into their roles.
- Promotes a culture of safety to ensure a healthy practice and living environment: Prioritize patient safety and well-being by promoting a culture of safety and respect.
- Maintains confidentiality and protects sensitive Protected Health Information (PHI) at all times: Ensure the confidentiality and security of patient information at all times.
- Graduate of an approved school for practical or vocational nursing is required.
- Must be currently licensed by the State Board of Nursing.
- Experience is preferred in rehabilitative or geriatric nursing.
- Maintains current BLS/CPR certification (All SNFs and Behavioral Centers; ALFs/ILFs as required by state).
