Join Our Team as a Home Health Care Professional

We are seeking a skilled and compassionate Home Health Care Professional to join our team at Optum Home & Community Care. As a key member of our team, you will play a vital role in delivering high-quality patient care and improving health outcomes in the comfort of patients' homes.

About the Role

This is a unique opportunity to work in a dynamic and patient-centered environment, where you will have the chance to make a meaningful difference in the lives of our patients. As a Home Health Care Professional, you will be responsible for conducting in-home assessments, evaluating patients' medical history, and providing comprehensive physical exams. You will also perform evidence-based practice screenings, identify diagnoses, and formulate treatment plans to ensure the best possible outcomes for our patients.

Responsibilities
  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing (as appropriate)
  • Identify diagnoses and formulate treatment plans to ensure the best possible outcomes for patients
  • Work collaboratively with patients, families, and healthcare teams to develop and implement individualized care plans
  • Provide education and support to patients and families to promote healthy behaviors and self-management
Requirements

To be successful in this role, you will need to have a strong foundation in clinical care, excellent communication and interpersonal skills, and the ability to work independently in a fast-paced environment. You will also need to be able to travel up to 85% within licensed states to complete home visit assessments.
