LPN Home Health Specialist

We are seeking an experienced Licensed Practical Nurse to join our home health team. As a LPN Home Health Specialist, you will be responsible for providing high-quality patient care and support in the comfort of their own homes.
Key Responsibilities:
- Conduct thorough assessments of patients to identify their health needs and develop personalized care plans.
- Administer medications and treatments as prescribed by physicians, and monitor patients' responses to care.
- Provide education and support to patients and their families on health maintenance, disease prevention, and management of chronic conditions.
- Collaborate with interdisciplinary teams to ensure comprehensive care and coordination of services.
- Document patient progress and maintain accurate medical records.
Requirements:
- Active, unencumbered LPN license in Oklahoma required.
- Minimum six months nursing experience, with home health experience preferred.
- Strong problem-solving and critical thinking skills, with ability to work independently and as part of a team.
- Excellent communication and interpersonal skills, with ability to build rapport with patients and families.
