We are seeking a highly skilled and compassionate Primary Care Physician to join our team at CenterWell Senior Primary Care. As a key member of our care team, you will be responsible for providing high-quality, patient-centered care to seniors in our community.
Responsibilities- Evaluate and treat patients in accordance with standards of care, utilizing evidence-based medicine and best practices.
- Collaborate with our interdisciplinary care team to coordinate patient services, including specialty referrals, hospital and SNF coordination, and durable medical equipment.
- Act as a medical expert and participate in daily huddles to ensure seamless care coordination and communication.
- Complete all medical record documentation in a timely manner, working with a quality-based coder to optimize coding specificity.
- Meet with the Regional Medical Director to discuss quality of care, review outcome data, and address policy and procedure issues.
- Participate in potential growth opportunities for new or existing services within the Center.
- Assure personal compliance with licensing, certification, and accrediting bodies.
- Current and unrestricted medical license or willingness to obtain a medical license in the state of practice.
- Graduate of an accredited MD or DO program.
- Excellent verbal and written communication skills.
- Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients.
- Fully engaged in the concept of 'Integrated team-based care' model.
- Willingness and ability to learn/adapt to practice in a value-based care setting.
- Superior patient/customer service.
- Basic computer skills, including email and EMR.
