Key Responsibilities- Coordinate Patient Care: Collaborate with medical center personnel to implement effective care plans, including scheduling consultations, referrals, and follow-up appointments.
- Provide Patient Education: Conduct effective teaching sessions with patients and families, documenting all education provided.
- Collect and Process Specimens: Collect, label, and send specimens to the laboratory for analysis, communicating and documenting lab results accurately.
- Assess and Document Patient Condition: Evaluate and document patients' physical and psychosocial condition, accurately assessing and documenting patient/family response to care.
- Administer Treatments and Medications: Administer patient treatments and medications in a safe, effective, and age-appropriate manner, in compliance with all policies, protocols, and regulations.
- Minimum Qualifications: Must possess the skill, knowledge, and ability essential to the successful performance of assigned duties, demonstrate customer service skills, and have a minimum of 6 months of experience working as an LVN in an acute care setting.
- Preferred Qualifications: Understanding of JCAHO requirements, knowledge of University and medical center organizations, policies, procedures, and forms, and bilingual skills in English and Spanish and/or Vietnamese.
