**Job Description**
We are seeking a highly skilled Post-Acute Care Physician Assistant to join our team at Curana Health. As a member of our interdisciplinary team, you will provide direct patient care services to seniors in the post-acute care setting.
Key Responsibilities- Conduct clinical interventional and episodic direct care visits
- Evaluate and treat chronic illnesses
- Coordinate care with collaborating physicians
- Assist with facility-wide illnesses
- Coordinate care with other disciplines
To be successful in this role, you must have a valid nurse practitioner or physician assistant license in the state of operations and current board certification in your field.
A Masters Degree in Nursing or Physician Assistant Studies is also required, as well as strong leadership, interpersonal, and problem-solving skills.
The estimated salary for this position is $125,000 - $185,000 per year, based on location and experience.
