Home Health Care Nurse Practitioner Opportunity

We are seeking a skilled Nurse Practitioner to provide high-quality patient-centered care in patients' homes. As a key member of our team, you will be responsible for delivering comprehensive care, maintaining accurate patient records, and scheduling appointments as needed.

Key Responsibilities:
  • Conduct initial patient screenings, physical examinations, and create assessment plans
  • Address urgent care needs of patients as required
  • Order, interpret, and analyze diagnostic tests, and determine the need for follow-up appointments and further treatment options
  • Prescribe and manage medications and treatments as appropriate
  • Deliver palliative care and end-of-life care to patients and families
  • Ensure quality care while maintaining company metrics related to patient engagement, HEDIS measures, and medical expense management
Requirements:
  • Experience in family practice, geriatrics, hospice care, palliative care, hospitalist, ICU, SICU, ER
  • Master's Degree in Nursing
  • Valid nurse practitioner or physician assistant license/Board Certification
  • NPI number as an NP
  • BLS/ACLS
  • Valid DL, Auto Insurance, willingness to drive to patients' homes/patient location
  • Competitive salary range: $103,000 - $137,000 plus bonus, benefits, and 4 weeks PTO
