Key Responsibilities:
  • Develop and implement individualized care plans with specific interventions and measurable patient outcomes.
  • Evaluate progress toward goals and revise care plans as needed.
  • Establish and implement personalized teaching plans for patients and their caregivers to promote independence at home.
  • Perform nursing procedures consistent with homecare protocols, utilizing standard homecare supplies and equipment.
  • Initiate and participate in regularly scheduled case conferences, including patient and family involvement in goal setting and care plan revision.
  • Maintain accurate clinical records, collect data, and prepare reports on activities.
  • Communicate regularly with the patient's physician, home healthcare team, and other key members of the care team.
